What to bring to your appointment
- Completed New Patient Forms
- Dental Insurance Card
- Valid Driver’s License.
If you’ve had X-rays taken in the last year, please have your last dental practice email them to firstname.lastname@example.org before scheduling your appointment. Our email is secure and HIPPA compliant.
If you’ve not had any x-rays taken in the past year, we will take them with our digital x-ray systems.
If more than a year since your last x-rays, digital photos of your teeth, mouth and smile will be taken. Afterwards, a thorough and comprehensive exam will be completed. This data is gathered to develop a treatment plan tailored to your specific needs. We will also listen to and address any of your concerns.

All payments are due at the time of service. Payment plans are available through Care Credit.

In an effort to serve every patient in need of dental care, we enforce a missed appointment policy. Please review the terms below and provide our office with a twenty-four-hour notice of appointments you will not be able to attend.
- One appointment missed without notice – Forgiven.
- Second appointment missed without notice – Patient will be removed from our patient roster.
- One forgiven missed appointment without notice.
- Second missed appointment without notice will be charged a fee of $25.00
- Upon the third missed appointment without notice, the patient will be removed from our patient roster.
